Intracerebral Hematoma Following Evacuation of Chronic Subdural Hematoma



Abstract
Among 78 cases of chronic subdural hematomas treated by multiple burble trephination, 5 cases of intracerebral hematomas, which were manifested by rapid deterioration of consciousness and focal neurological findings occurring usually immediately after original procedures (but, in 2 cases, 24 hours and 48 hours after), were reported.
Possible pathogenic mechanisms include hemorrhage into previously undetected areas of contusion, sudden increase in cerebral blood flow combined with faulty autoregulation, and damage to parenchyma vessels secondary to rapid intra- or postoperative shift of the intracranial contents.
Perhaps this devastating complication can be avoided if twist-drill craniotomy with continuous catheter drainage is used for treatment of chronic subdural hematomas.
